How To Order
** Do not order this service if you are pregnant **
- Click on the ‘Order HrC Genomics‘ button, which will take you to a page where you can place your order.
- Select the number you wish to order and add it to your basket.
- Proceed to Checkout and make a payment.
- Once your payment has been accepted, a ‘Thank You’ page will be displayed containing a link to the Patient Information & Consent Form.
- Once a Patient Information & Consent Form has been completed and submitted, a Booking Page will be displayed where you can book an appointment with a Phlebotomist for the blood draw.

Phlebotomist appointments are only available Monday to Thursday (barring Bank Holidays).
Patient Information & Consent Form
The Patient Information & Consent Form is made up of the following sections:
- Eligibility Criteria – determines eligibility or if your appointment needs to be rescheduled.
- Personal Information – basic information required for identification purposes and also for including on HrC Genomics Report.
- Clinical Details – cancer related, required as part of the overall data analysis.
- Medical History – general medical details required as part of overall data analysis.
- Family History – cancer related, required as part of the overall data analysis.
- General Health Questions – required as part of the overall data analysis.
- Patient Consent Form – needs to be accepted before the HrC Genomics Service can be provided.
